The Support at Home Program begins 1 November 2025 and is designed to help older Australians stay independent at home with coordinated, flexible care. This program replaces Home Care Packages, offering a simpler, more transparent system.
Support at Home provides:
A co-designed service agreement tailored to your goals.
An individualised budget so you can see exactly how funds are allocated and spent.
The freedom to choose what services you want and when.

Minor home repairs, garden maintenance, and safety modifications. Installation of ramps, grab rails, and assistive equipment.
Assistance with bathing, showering, dressing, grooming, and mobility. Help with communication and daily living activities.
Cleaning, laundry, and household tasks. Meal preparation and support with nutrition and hydration.
Nursing care (registered/enrolled nurses, nursing assistants). Medication management, wound care, continence care, and skin integrity. Education and specialist service linkage.
Physiotherapy, occupational therapy, podiatry, dietetics, exercise physiology. Speech therapy, counselling, and other therapeutic services. Techniques for self-management and maintaining physical and cognitive abilities.
Assistance to attend appointments, shopping, and community activities. Social engagement programs to reduce isolation.
Yes. From 1 November 2025, the Support at Home Program replaces Home Care Packages.
If you currently have a Home Care Package, you will transition to Support at Home under the new system.
To access a Support at Home package, you must:
✔ Be registered with My Aged Care
✔ Have a current aged care assessment
Funding is based on your assessed needs through My Aged Care (MAC).
Funds are allocated directly to providers to deliver your chosen services.
Package amounts vary depending on your care level and personal circumstances.
Contributions are not means-tested, but you’ll pay a fair share toward your services based on your financial situation.
Under the new Aged Care Act, a lifetime cap of $130,000 on non-clinical contributions applies, with daily and monthly limits for certain services.
Financial assistance may be available for those who need it.
